ABSTRACT

Objective To investigate the status of professional autonomy and turnover intention of clinical specialist nurses and analyze the correlation between them. Method A total of 200 clinical specialist nurses in Shandong Province were selected for questionnaire survey. Nursing job behavior scale and turnover intention were investigated. Results The total score on clinical autonomy was(172.26±18.31)points,with an average of(4.92±0.56).The total score on turnover intention was(15.84 ± 3.47)points, averaged (2.64 ±0.65). There was a negative correlation between the professional autonomy and turnover intention (P<0.01). Conclusion Nursing administrators should take measures to improve the professional autonomy of specialist nurses in order to reduce their turnover rate and stabilize the nursing team.

ABSTRACT

Objective To investigate the status of disability acceptance and post traumatic growth in patients with urinary diversion of bladder cancer and to analyze the correlation between them. Methods One hundred and fourteen cases with urinary diversion of bladder cancer were investigated with Acceptance Disability Scale and Post Traumatic Growth Inventory. Results The score of total score of disability acceptance and post traumatic growth of bladder cancer patients with urinary diversion and abdominal wall ostomy were (177.49 ± 38.28) and (56.13 ± 14.56) points respectively. There was a positive correlation between the total score and each dimension (r=0.274-0.413, P <0.05). Conclusions There is a clear correlation between disability acceptance and post traumatic growth in patients with urinary diversion of bladder cancer. Targeted measures should be taken to improve patients'disability acceptance so as to enhance their post traumatic growth and promote their quality of life.

ABSTRACT

Objective: To compare the difference of Response Evaluation Criteria in Solid Tumors 1.1 (RECIST 1.1) and modified Re-sponse Evaluation Criteria in Solid Tumors (mRECIST) in the treatment of hepatocellular carcinoma (HCC) after stereotactic body radio-therapy (SBRT). Methods:From Janurary 2014 to August 2015, thirty-five patients with HCC treated with SBRT were included in De-partment of Radiation Oncology and Integrative Oncology of Navy General Hospital of PLA, and SBRT efficacy was evaluated based on RECIST 1.1 and mRECIST criteria. Results:Under RECIST 1.1, one patient had complete response (CR), 20 had partial response (PR), and 11 achieved stable disease (SD) at three months. Three patients had progressive disease (PD). The overall best response rate (CR+PR) was 60%. In comparison, under mRECIST, 10 patients had CR, 16 had PR, and 6 achieved SD at three months. Three patients had PD. The overall best response rate was 74.28%. The statistical analysis showed that Kappa=0.402 (χ2=43.3, P<0. 001) was less than 0.75 but greater than 0.4, indicating that it had not reached the two diagnostic criteria of consistency degree of satisfaction. According to the mRECIST criteria, the objective remission group (CR+PR) was superior to the nonobjective remission group (SD+PD) in progression-free survival (P<0.001). Conclusion:For unresectable HCC, mRECIST may be more useful than RECIST 1.1 in evaluating HCC response to SBRT.

ABSTRACT

Objective To collect the hot spots, development trends, high yield authors and their affiliated institutions by analyzing the papers on pancreatic cancer chemotherapy in recent 5 years. Methods The papers on pancreatic cancer chemotherapy were downloaded from PubMed. Their high frequency subject headings extracted by BICOMB were analyzed by bibliometric double clustering analysis and strategic coordinate analysis. The high yield authors were analyzed by co-authorship group analysis. Results The current studies on pancreatic cancer chemotherapy in foreign countries were focused on the pathological features of pancreatic duct adenocarcinoma before and after chemotherapy, drug therapy of pancreatic cancer according to its genetics, metabolism and pharmacology of antitumor drugs, surgical treatment combined with radiotherapy and chemotherapy for pancreatic cancer. The high yield authors could be divided into two large groups from Japan and USA and a number of small groups. Conclusion New breakthrough points should be found from the matured topics, the topics with a further developmental space should be greatly concerned, and cooperation should be strengthened between study groups or teams.

ABSTRACT

[ABSTRACT]AIM:ToinvestigatetheroleofquercetinintheapoptosisofhumanbreastcancercelllineMCF-7 and the association with Fas/Fas ligand (FasL) pathway.METHODS: The apoptosis model of MCF-7 cells was estab-lished by the induction with quercetin .The morphological characteristics of apoptotic MCF-7 cells were observed under transmission electron microscope .The apoptotic rates and alternation of mitochondrial membrane potential (Δψm ) in the MCF-7 cells were measured by flow cytometry using fluorescein labeled Annexin V-FITC/PI and JC-1, respectively.FasL neutralizing antibody was applied to block the apoptosis .The expression of Fas/FasL on the cells was detected by immuno-fluorescence technique and flow cytometry , respectively.The influence of SB203580 (an inhibitor of p38 MAPK) on the expression of Fas/FasL was also examined by flow cytometry .The protein levels of p 38 MAPK and p-p38 MAPK were de-termined by Western blot .RESULTS: The phenomenon of nuclear condensation and marginalization in the MCF -7 cells treated with quercetin at 80.0 μmol/L for 48 h was observed under transmission electron microscope .Compared with the control cells , theΔψm was decreased by 17.4%, 44.3% and 68.9% in the MCF-7 cells treated with quercetin at 80.0μmol/L for 24 h, 48 h and 72 h, respectively .The apoptotic rates of MCF-7 cells treated with quercetin at 80.0 μmol/L for 24 h, 48 h and 72 h were (10.2 ±3.3)%, (28.9 ±7.5)%and (39.2 ±8.9)%, respectively.However, the apop-totic rates were decreased to (8.2 ±2.8)%, (19.2 ±5.3)% and (22.5 ±6.9)% after the cells were pretreated with FasL neutralizing antibody , respectively .When MCF-7 cells were treated with quercetin for 24 h, 48 h and 72 h, Fas/FasL expression rates were increased in a time-dependent manner , which were largely inhibited by SB203580.The protein level of p38 MAPK was not changed obviously , but the protein level of p-p38 MAPK was significantly increased at 48 h and 72 h.CONCLUSION: Quercetin up-regulates the expression of Fas/FasL on MCF-7 cells, and induces apoptosis via Fas/FasL pathway .Meanwhile , p-p38 MAPK is potentially critical signaling molecule for up-regulating the expression of Fas/FasL.

ABSTRACT

Objective To explore the effects of intracellular calcium concentration ([Ca2 + ]i )and signal transducers and activators of transcription 3 (STAT3 )signaling pathway on the invasion of Vibrio vuknificus (Vv )clinical isolates B2 into dendritic cells (DC).Methods The co-culture model of Vv B2 and DC 2.4 was constructed.Apoptosis and necrosis rates in different invasion phases were detected by flow cytometry.The calcium fluorescence probe Fluo-8-AM was used to detect the change of [Ca2 + ]i ,and Western blot and immunofluorescent techniques were used to explore the relative molecular expressions and intracellular distributions of STAT3 and phosphorylation STAT3 [p-STAT3 (705 )].Data were analyzed by t-test.Results After co-culture ofVv B2 and DC 2.4,the apoptosis rates of 1 ,2,3 and 4 h were (13.10±4.72)%,(30.10±3.52)%,(46.20±15 .61)% and(31 .00 ±19.10)%,respectively.The differences were statistical significant between Vv standard strain (1 .1758)and Vv B2 (t=4.30,22.33, 4.30 and 4.30,respectively;P =0.040,0.002,0.040 and 0.040,respectively).The necrosis rates of 1 , 2,3 and 4 h in co-culture of Vv B2 and DC 2.4 were(19.70 ±3.50 )%,(39.20±4.60 )%,(40.90 ± 13.80)% and (62.10 ± 8.20 )%,respectively.The differences were statistical significant between Vv 1 .1758 andVv B2 (t=9.93,14.09,4.30 and 14.09,respectively;P =0.010,0.005 ,0.049 and 0.005 , respectively).Compared to Vv 1 .1758,Vv B2 induced more apparent cell apoptosis and necrosis within the same time.By fluorescence microscope,intensity of calcium was enhanced with the rising of apoptosis. Although the total expression of STAT3 increased,the relative molecular expression of p-STAT3 (705 ) decreased.There was no significant difference in its distribution in cells compared to the blank group. Conclusion The rapid apoptosis and necrosis of DC during Vv B2 invasion into DC may be caused by increasing [Ca2 + ]i and inhibiting phosphorylation of p-STAT3 (705)simultaneously.

ABSTRACT

AIM:To investigate the role of damaged mitochondria in dendritic cell ( DC) apoptosis induced by Vibrio vulnificus (Vv) and its possible mechanism.METHODS: DC2.4 cells were co-cultured with Vv 1.1758 strain. Fluorescent probes DCFH-DA and Fluo-8-AM were used to detect reactive oxygen species ( ROS) and intracellular Ca 2+concentration in the invaded cells , respectively .The cellular apoptotic rates and mitochondrial membrane potential (Δψm ) were measured by flow cytometry.The expression of nuclear factor-kappa B p65 (NF-κB p65) and tumor necrosis factor-al-pha (TNF-α) was detected by Western blotting.RESULTS:Vv 1.1758 induced DC2.4 cell apoptosis.Vv 1.1758 bacte-ria invaded into the DC2.4 cells by binding with cellular membrane though the end of the body .In the invaded DC2.4 cells, the visible mitochondrial damage, elevated ROS and intracellular Ca2+levels, and declinedΔψm were presented.Af-ter 1 h of co-culture, NF-κB p65 began to rise and reached the peak at 5 h, and then slightly decreased at 6 h.The TNF-αlevel increased after 2 h of co-culture and reached the peak at 6 h.CONCLUSION:The damaged mitochondria play an important role in DC apoptosis induced by Vv , and its possible mechanism may associate with the elevation of ROS and in-tracellular Ca2+level, and the declined Δψm.Meanwhile, NF-κB p65 and TNF-αare potential critical signaling molecules in the process of apoptosis .

ABSTRACT

Objective To review the clinical features of lung nodule and the common misdiagnosed reasons and diseases. Methods A total of 44 literature on misdiagnosed lung nodule in the latest ten years were reviewed and their clinical characters, diagnosis methods were analyzed. Results Forty-four relative literatures were found, in which there were 319 misdiagnosed cases. The median of onset age was 43 years. Female patients were more than male. Cough, chest distress and fever were primary symptoms. 46. 39% of the patients were characterized with hilus of lung lymphadenectasis symmetrically. The most common misdiagnosis were tuberculosis, lung cancer and lymphadenoma ( 53.61%, 25. 71% and 10. 34% ). Majority of patients were correctly diagnosed by biopsy, especially through transbronchial lung or mucosa biopsy or lymphaden biopsy. Conclusion Clinicians should improve the understanding on lung nodule and comprehensively analyze and differentiate it with other diseases to reduce the misdiagnosis.

ABSTRACT

To study the effects of mouse cytomegalovirus (MCMV) on the in vitro maturation, fertilization, cleavage and blastula formation of mouse oocytes, the immature oocytes were infected in vitro by MCMVs of different dosages (100 TCID(50), 10 TCID(50) and 1 TCID(50)). The oocytes were then observed for in vitro maturation, fertilization, cleavage and blastula formation and the ultrastructural changes after the culture with the viruses. Our results showed that no significant differences were found in IVM, IVF, cleavage and blastula formation among the groups treated with of virus of various dosages. And ultrastructural abnormality was observed in the oocytes treated by 100 TCID(50) of viruses. It is concluded that MCMV did not have any conspicuous effects on IVM, IVF, cleavage and blastula formation of murine immature oocytes.

ABSTRACT

ABSTRACT

With the exploitation of marine resources and environmental deterioration, perilous events in the sea are gradually increasing. Seawater drowning is one of the most important causes of death. Researches on seawater drowning have attracted more and more interests in recent years, and certain progresses have been achieved in scientific research and clinical treatment. In the aspects of theory and establishment of a model, a renewed concept of seawater drowning-induced acute lung injury (SWD-ALI) has been acquired, the animal model of SWD-ALI has been successfully reprodaced for elucidation of the pathological and pathophysiological changes in lung tissue. Multiple results of molecular biological research on SWD-ALI have contributed to illuminating the pathogenesis involved. It is indicated that, compared with freshwater-induced lung injuries, SWD-ALI is severer in clinical picture, and it might develop to a peculiar seawater drowning-induced acute respiratory distress syndrome (SWD-ARDS) if no prompt and effective treatment is employed. For the clinical treatment for SWD-ALI, comprehensive therapies based on mechanical ventilation have been approved. The scientific outcomes mentioned above have laid a foundation for the full realization of SWD-ALI and more effective rescue of SWD-ALI patients.

ABSTRACT

Objective To explore the dynamic changes in apoptosis of polymorphonuclear neutrophil (PMN) during the course of seawater drowning-induced acute lung injury (SWD-ALI), and the possible roles of cytokines in the process. Methods Thirty-six NZ rabbits were randomly divided into 6 groups by using a random number table: control group (0h, as baseline value) and 5 seawater drowning groups (S1h, S3h, S6h, S12h and S24h). Blood gas analysis and hemogram were observed at different time points. The ratio of wet to dry lung weight (W/D) and lung permeability index (LPI) were calculated. The changes in PMN apoptosis were analyzed by flow cytometer. The concentrations of tumor necrosis factor alpha (TNF-?), interleukin-1 (IL-1 ) and IL-10 in serum were detected by ELISA. Meanwhile, the pathological changes in lung tissue were examined with hematoxylin-eosin (HE) staining, and lung pathologic scores (LPS) were evaluated. Results The W/D ratio reached its peak value at 3h, while LPI at 6h. The histopathologic findings indicated that inflammatory cell infiltration was most massive at 6~12h. LPS increased at 1h, significantly higher than that of control group (P0.05). Conclusions The apoptosis of PMN, the main inflammatory effector cells, is significantly inhibited in the early stage of SWD-ALI. The serum cytokines, TNF-?, IL-1 and IL-10, play important roles in the pathogenesis of SWD-ALI, but have no evident effects on the inhibition of PMN apoptosis.

ABSTRACT

Objective:To observe the effect of partial liquid ventilation (PLV) on gas exchange,pulmonary compliance and blood dynamics in the pulmonary edema rabbits after seawater drowning(PE-SWD). Methods:Totally 42 healthy male New Zealand rabbits were randomly allocated into 3 groups (n=14);each group was treated with various methods after PE-SWD model was established:PE group with no treatment,CMV group with conventional mechanical ventilation and positive end-expiratory pressure(PEEP),PLV group with CMV,PEEP and FDC.After being treated for 3 h,blood gases,pulmonary mechanics,hemodynamics,pulmonary histopathology and survival time were observed.Results:In PLV group,PaO 2 increased obviously and reached (262.84?64.33) mmHg (1 mmHg=0.133 kPa), airway pressure decreased by 23.4%,pulmonary static compliance increased by 31.3% and TNF-? decreased by 24.9%.HR and MAP showed no significant changes after treatment compared with PE group.Conclusion: Pulmonary gas exchange and compliance are improved significantly during PLV in the PE-SWD rabbits without obvious influence on hemodynamic status, although the survival time of PLV group is less than that of CMV group.

ABSTRACT

Objective To observe the effect of partial liquid ventilation (PLV) and conventional mechanical ventilation(CMV) on gas exchange in lung injury due to pulmonary edema as a result of seawater drowning in rabbit. Methods Twenty-four healthy male new Zealand rabbits were randomly divided into three groups ( n =8). Pulmonary edema was produced in all rabbits by drowning in sea water. Rabbits of PE group received no treatment,that of CMV group were treated with conventional mechanical ventilation and PEEP,and that of PLV group with CMV,combined with PEEP and FDC. All rabbits were treated for 3 hours. Blood gases,TNF-?in plasma and pulmonary histopathology were determined during the experiment. Results In PLV group,PaO_2 was increased obviously,inflammatory exudate was reduced,and alveolar and interstitial edema was alleviated. ABSTRACT

Objective To study the effect of pelvic floor electric stimulation on bladder function in goats. Methods 18 goats were randomized into the stimulating and the control group. Results In the stimulating group,the bladder capacity significantly increased by 24%,the voiding pressure significantly declined by 24% and the metabolic level of the detrusor cells is significantly lower. Conclusions Electric stimulation of the pelvic floor would inhibit bladder contraction and may be helpful in the management of instable bladder and stress incontinence.
